Parental Consent Form

This form must be printed, signed, and uploaded to all Youth Grant applications for those applications to be considered during the grant review process.

The success of your son's or daughter's workshop/training depends, in part, on your support. The total value of each grant award is up to $750.00 towards a workshop/training opportunity and supply costs. The board has the discretion to increase the grant award given certain circumstances.

You will be expected to help provide the following:

• Although some materials are covered by the grant, additional materials may be necessary, such as paints, canvas, dance shoes, sheet music, camera equipment, musical instrument, etc.
• Transportation to and from where lessons or the workshop/training will take place.
• Past experience has shown that the students who make the most progress receive support and encouragement to practice at least three hours per week at home.
